Congratulations on your nomination!
Nominees will be emailed a link to their application along with a password. If you believe you have been nominated, but have not received an email or your password is incorrect, please contact us and your Athletic Director.
Before you Begin
Make sure you have reviewed the nomination process and requirements. You will be asked questions in about your Athletic Achievements, Extra Curricular Activities, Leadership Experience, and Community Service.
The application is comprehensive and may take an hour or more. You may save the application at any time. Simply click “Save and Resume Later” at the bottom of the form.
- Students must be nominated by their school’s Athletic Director.
- A password is required to access the application.
- Applications received after stated deadlines for each sport will not be eligible for the Champions Scholarship Award.
Please have ready the following:
- Official high school transcript (required)
- School letter of recommendation(required)
- Community letter of recommendation (optional)
